The Surgeon makes a cautious cut using a 15 blade. Sebaceous cysts can often spurt out liquid or pus either when the Local Anaesthetic is being injected or the incision is made. Thus goggles can be worn and also a gown. Note here gauze is being used to reduce the risk of the fluid squirting into the face. The first cut is very superficial - to test the adequacy of the anaesthesia and assess the patient's response. Note that the hands are steadying the scalpel.
